Accessibility Intake Form

Please use this form to start an initial request for a new accommodations plan with Sonoran University's Accessibility Office. Answer all questions as accurately and thoroughly as possible so that we may best determine what is needed. Request for accommodations should be requested as soon as known but no later than 2 weeks in advance of the start of the term. Please be aware that accommodations impacting practical, hands on and skill-practicing assessments need to be in place at least 2 terms in advance due to the complex nature of scheduling and administering those exams. Upon completion of the form, please feel free to request an Intake Meeting with the Accessibility Services Administrator at dos@sonoran.edu or Accessibility@sonoran.edu

NOTE: Accommodations do not go into effect until approved and signed off by both the University and the student. No prior accommodations plan is required and temporary accommodations are available while obtaining documentation.

 

Accessibility Change Form

If your accommodations are not addressing all the barriers to education you are experiencing, or your underlying medical condition has significantly changed, you may request a review and adjustment of your current plan with the accessibility change form. Requests need to be made as soon as known but no later than 2 weeks in advance of the start of the term. Adjustments to accommodations impacting practical, hands on and skill-practicing assessments will be implemented as quickly as possible but full implementation may require delay of the assessment / course, until requirements can be met, due to the 2 term scheduling restrictions on certain assessments. Please do not delay identification of changes to avoid this scenario. Upon completion of the form, please feel free to request an Intake Meeting with the Accessibility Services Administrator at dos@sonoran.edu or  Accessibility@sonoran.edu

Accessibility Supporting Documentation Form

Use this form to submit medical documentation for previously requested plan or adjustment. Supporting documentation must including an original signed letter from a licensed physician identifying the diagnosis of a specific disability and the related required accommodations. Testing Accommodations Request Form

Complete a Testing Center Appointment Request form for utilizing testing accommodations, at least 4 days in advance, for all tests, quizzes, or other timed & graded assessments will be taken in the Testing Center only. Online exams, practical exams and lab exams require direct coordination with course faculty. Do not use this for for those assessments. Due to space limitations, the Accessibility office cannot honor requests with less than 4 days notice. Students will need to take their exam in the classroom without accommodations. If you have any questions about the process, please do not hesitate to ask.

NOTE: Your exam may be administered in the Testing Center by non-ADA staff, therefore it is critical that you review the accommodations you are set to receive to ensure they meet your needs/Accommodation plan prior to arriving/sitting for the examination. Please reach out to the Accessibility Office immediately to address any changes in your Accommodation plan.
