SCNM Accessibility Office Forms:

Accessibility Intake Form

The form below is for students who have not previously been registered with SCNM’s Accessibility Office. If you are applying for accommodations or services, please answer the questions as accurately and thoroughly as possible so that we may process your request. Request for accommodations should be requested as soon as possible but no later than 1 week in advance of the start of the term. Please be aware that accommodations impacting practical, hands on and skill-practicing assessments need to be in place at least 2 terms in advance. Upon completion of the form, a representative from our office will contact you within 2 business days. If you have not heard from us after that time, please contact our office at 480.222.9203 or email us at adaoffice@scnm.edu .

Accessibility Change Form

The form below is for students who have previously been registered with SCNM’s Accessibility Office and wish to add, substitute, or remove approved accommodations. Please answer the questions as accurately and thoroughly as possible so that we may process your request. Request for modifications can be requested at any time but no later than 1 week in advance of the start of the term. Please be aware that accommodations impacting practical, hands on and skill-practicing assessments need to be requested at least 2 terms in advance so please submit changes as soon as known. Upon completion of the form, a representative from our office will contact you within 2 business days. Accessibility Supporting Documentation Form

The form below is for students who have previously been registered with SCNM’s Accessibility Office. If you are requesting  accommodations for the first time or requesting a change to your current accommodations please use the appropriate form above to submit your supporting documents.

Supporting documentation must including an original signed letter from a licensed physician identifying the diagnosis of a specific disability and the related required accommodations.
